HCPCS Q3014 Telehealth originating-site facility fee
The 2026 maximum facility-fee amount increased from $31.01 to $31.85. Medicare pays 80% of the lesser of the actual charge or $31.85, subject to deductible and coinsurance.
HCPCS G0136 Social Determinants of Health Risk Assessment
CMS considered removing G0136 but retained it on the Medicare Telehealth Services List. Practices may continue reporting it through telehealth when all coverage, timing and documentation requirements are met.
Subsequent inpatient telehealth services
CMS permanently removed frequency restrictions affecting subsequent inpatient services, including the relevant inpatient E/M and telehealth consultation code families.
Subsequent nursing-facility services
The previous limits on how frequently subsequent nursing-facility care could be furnished through telehealth were permanently removed.
Critical-care consultations
Telehealth frequency limitations for critical-care consultations were also permanently removed beginning January 1, 2026.
Virtual direct supervision
Real-time, two-way audio-video supervision is now permanently permitted for many qualifying services. Audio-only supervision does not meet this standard, and services carrying a 010 or 090 global surgical indicator are excluded from the expanded policy.
Teaching physician virtual presence
A teaching physician may participate virtually when the patient, resident and teaching physician are connected in a three-way telehealth visit from separate locations.
RHC and FQHC telehealth
CMS continued payment for medical telehealth services furnished by Rural Health Clinics and Federally Qualified Health Centers through December 31, 2026.
